Standard form that is required by most federal agencies when applying for federal funds.
- General Assurances
A list of assurances that the applicant/recipient certifies compliance in the acceptance and use of federal funds. - Budget Detail Worksheet
Budget outline that may be used as a guide to assist in the preparation of the budget and budget narrative. The budget and budget narrative may be submitted by using this form or another format of choice, but the required information (including the budget narrative) must be provided. Any category of expense not applicable to your budget may be deleted. - Disclosure of Lobbying Activities
The filing of a form is required for each payment or agreement to make payment to any lobbying entity for influencing or attempting to influence an officer or employee of any agency, a member of Congress, an officer or employee of Congress, or any employee of a member of Congress in connection with a covered federal action. - Financial Status Report (SF 269A)

Signing this form certifies compliance with certification requirements under 28 CFR Part 69, "New Restrictions on Lobbying," and 28 CFR Part 67, "Government-Wide Debarment and Suspension (Nonprocurement) and Government-Wide Requirements for Drug-Free Workplace (Grants)."
